For LW settings, see Manual port forward instructions & sample image.

For your router, both UDP & TCP should be same port. I've heard it's best to do a separate port forward rule for each one but it's not necessary.

And remember, the most important step in port forwarding is the very first one, settpng up a static ip PortForward.com - Free Help Setting up Static IP. If you don't do this at very first step then port forwarding rule won't work as your ip will change & port forwarding is based on your ip.

No it's not necessary to use the same Listening port as downloading one. Listening port is the most important to port forward. Unless your router has an extreme NAT firewall, the main port may not be affected. But it's a lot easier to keep both ports the same.
