As far as I am aware, music, film or any other files do not ask you to fill in a survey. In fact, no one else in this forum has complained of or come across this before. I can only presume that your PC has been infected with with some sort of spyware. Therefore, I would suggest that you run Spybot (or some other anti-spyware app) to check and clean your HD.

Has limewire changed, is it no longer free?

Limewire Basic is still free and apart from the necessary upgrades and updates it still has the same primary functions.
